## Description

Centene is hiring a Care Manager to support care management activities for members with behavioral health and physical health needs from birth through age 64. The role performs member outreach by phone or home visit, coordinates care and service plans with healthcare and community providers, documents care needs and records, provides education on benefits and resources, and makes referrals as needed. Applicants may work remotely from anywhere within Missouri on a Monday–Friday, 8 a.m.–5 p.m. schedule and must have a high school diploma or GED plus 1–2 years of related experience. Pay is $17.84–$28.02 per hour, with health insurance and other benefits mentioned.
