## Description

The Case Manager is a 1099 independent contractor providing case management services to The Commitment House clients. Responsibilities include completing prior authorizations, assurance forms, needs assessments, treatment plans, four monthly contacts, monthly summaries, and maintaining accurate client records; managing no more than 30 active clients; complying with applicable laws, ethical standards, and company policies; and maintaining required training, certifications, supervision, continuing education, and liability insurance. The role sets its own schedule, requires daily check-ins, and is paid 50% of collected case-management fees on the first Friday of each month. The contractor is responsible for taxes, self-employment taxes, health, liability, and professional insurance, and is not eligible for employee benefits.
