## Description

Astor Services is seeking a full-time Case Planner to provide in-home counseling services to families in need. The Case Planner will conduct weekly change-focused interventions, assess child and family well-being, collaborate with external service providers, and develop service plans. This role involves a caseload of 12-14 families, requiring frequent face-to-face interactions and some late-night availability.
