Wisconsin NEMT Provider NMotion Expands Hiring Statewide

The Expanding Demand

NMotion Medical Transportation, a Madison-based non-emergency medical transport provider, announced a statewide hiring initiative as demand for patient transportation services continues accelerating across Wisconsin. The company completes hundreds of trips monthly, helping patients safely reach medical appointments, clinics, and hospitals throughout Dane County and beyond.

"A Madison-based medical transport company is expanding hiring efforts as demand for non-emergency medical transportation continues to grow across Wisconsin," Joe Durruthy, NMotion's owner and CEO, told Spectrum News in early January 2026. His team transports patients to essential appointments, ensuring those without reliable transportation or facing mobility challenges can access critical healthcare services.

The expansion reflects broader industry trends. The non-emergency medical transportation (NEMT) sector is experiencing significant growth, with the global market projected to reach $13.87 billion by 2030 according to industry analyses. The market maintains a steady 7.05% compound annual growth rate (CAGR) through 2025, driven by an aging Baby Boomer generation, rising chronic health conditions, and improved government funding through Medicaid and Medicare programs.

In the United States specifically, the NEMT market grew from $10.76 billion in 2024 to reach an estimated $11.80 billion in 2025, with projections showing continued expansion to $17.99 billion by 2030 at an 8.81% CAGR. Mordor Intelligence data confirms this trajectory, noting that North America leads the global NEMT market due to robust healthcare infrastructure and government transportation benefits.

Wisconsin stands among states with highest NEMT utilization. According to CMS (Centers for Medicare & Medicaid Services) data analyzed by Health Management Associates, Alaska, Minnesota, Arizona, Maine, and Wisconsin had the highest percentages of Medicaid beneficiaries using NEMT services, with rates reaching up to 11% in Alaska during 2021. Approximately 3-4 million Medicaid beneficiaries used NEMT services annually between 2018 and 2021, representing 4-5% of all Medicaid enrollees nationwide.

From Street Corners to Company Founder

Durruthy's journey to founding NMotion in 2015 represents a remarkable transformation. Born in 1984 while his Afro-Cuban father served prison time for assault with a deadly weapon, Durruthy grew up on Madison's southside surrounded by gang members and drug dealers who served as the only visible role models. His mother struggled with crack addiction. His childhood neighborhood on Eric Circle exposed him to street life from an early age.

"In 1993-94 we moved to Eric Circle and looked up to the older guys selling drugs," Durruthy recalled in a 2020 UMOJA Magazine profile. "My cousin and brother were selling, too. Based off what that looked like, me and my peers would break a piece of drywall off the walls and sell it like it was crack. It worked. We basically ripped them off for $20 a bag."

An attempt at "normal" employment at a fast-food restaurant resulted in small paychecks that couldn't compete with street income. Durruthy returned to selling narcotics. But having a child and self-will ultimately set him on life's right path. Using earnings from party promoting, he purchased a 17-seat shuttle bus from Minnesota to transport people to and from his events.

After naming the venture NMotion, Durruthy and a friend worked day and night building a trustworthy and reliable reputation. A tip urging him to expand into shuttling patients to and from medical appointments proved transformative. It happened that one of four buses he purchased was equipped with a wheelchair lift. The advice paid off.

Now father of four with a fiancée, Durruthy told UMOJA Magazine that NMotion Medical Transportation grosses between $30,000 to $45,000 monthly. "We are the VA Hospital's number one vendor in Madison," Durruthy explained. "We also work with UW Health, Meriter, St. Mary's Hospital, and multiple dialysis companies and nursing homes."

Valentino Varese, president of NMotion Medical Transportation, emphasized the company's operational philosophy: "It's important to understand and take care of the staff, because the drivers take care of the passengers." Leadership ensures drivers' vehicles are fueled every morning and preheated before trips on chilly Wisconsin days, preparing them for their shifts.

The Hiring Initiative

NMotion currently seeks drivers, dispatchers, and support staff as the industry continues growing. Applicants need a valid driver's license and clean driving record. Durruthy noted the company could help new hires get started anywhere in Wisconsin, removing geographic barriers to employment.

Current job postings advertise a $1,000 hiring bonus for drivers. Positions offer part-time employment with full-time potential based on performance and business needs. Two shifts are available, with typical work hours ranging between 4:00 AM to 2:00 PM or 10:00 AM to 8:00 PM. Compensation ranges from $17 to $25 hourly according to listings on ZipRecruiter.

Key responsibilities include providing safe and timely transportation for passengers, operating vehicles equipped for stretcher, wheelchair, and ambulatory transport, assisting passengers with boarding and disembarking as needed, and communicating effectively with dispatchers using GPS mapping and dispatching applications. No previous medical experience is required, making positions accessible to workers from various backgrounds.

Lonell James III, a father of three who works as both driver and dispatcher for NMotion, highlighted flexibility as one of the biggest benefits. The company's scheduling accommodates workers balancing family responsibilities, education, or other commitments. That flexibility proves crucial in attracting talent amid the tight labor market that characterizes 2025-2026 employment conditions.

The Wellness Focus

Beyond flexible scheduling, NMotion emphasizes employee wellness through on-site exercise facilities and health-focused perks. This wellness dedication addresses the physical demands of medical transportation work, which involves assisting patients with mobility challenges, operating specialized equipment, and spending extended hours on the road.

The company's wellness approach aligns with broader NEMT industry recognition that driver retention requires addressing occupational stressors. Annual turnover exceeds 64% in several US NEMT markets according to Mordor Intelligence analysis, compounded by competition from parcel delivery and rideshare firms promising flexible hours. NEMT roles require wheelchair-securement training, HIPAA familiarity, and background checks, shrinking the qualified labor pool.

By investing in driver wellbeing through exercise facilities, vehicle maintenance, and supportive management, NMotion aims to buck industry turnover trends. The strategy recognizes that satisfied, healthy drivers provide better patient care, maintain safer driving records, and stay with employers longer—reducing recruitment and training costs while improving service consistency.

The Human Connection

Driver John Carr emphasized NMotion's mission extends beyond mere transportation logistics. "We do it out of the love of what we do," Carr explained. "We really just love helping people and making sure they can get to these important appointments and get back home."

That human element proves essential in NEMT services. Many passengers lack regular socialization due to health complications preventing them from attending gatherings or maintaining active social lives. For some, the medical transport driver represents their primary human contact outside healthcare facilities. Building familiar relationships between passengers and drivers creates trust and comfort that improves the transportation experience.

"Many passengers become like family," Durruthy noted. That familial bond motivates the company's expansion plans. NMotion intends to honor its late founding members by expanding service routes across Wisconsin. "We'd like locations in Milwaukee, La Crosse and a lot of the upper-north areas," Durruthy said. "There's a lot of opportunity and potential up there."

The expansion targets underserved rural areas where transportation barriers significantly impact healthcare access. In rural Wisconsin, limited public transit, long distances between homes and medical facilities, and harsh winter weather create persistent challenges for patients needing regular appointments. NEMT services become essential infrastructure enabling healthcare access in these communities.

The Industry Landscape

The NEMT sector's growth stems from multiple converging factors. The aging Baby Boomer generation drives consistent demand for medical transport, especially for managing chronic health conditions that require regular appointments. The Insight Partners projects the non-emergency medical transportation market will reach $31.87 billion by 2031, growing from $16.71 billion in 2024 at a 9.7% CAGR.

Medicaid remains the dominant payer, commanding 52.34% of NEMT market share according to Mordor Intelligence. However, managed-care entities record the swiftest gains, with their 10.04% CAGR reflecting ongoing rollouts of statewide brokerage contracts tying per-member-per-month transport budgets to quality metrics. Private insurers have begun adding transport to employer policies to curb absenteeism, while Medicare Advantage plans moved from negligible coverage to 36% inclusion in 2025.

By vehicle type, wheelchair-enabled vans led with 43.71% of market share in 2024. Hybrid and electric vans are projected to advance at a 9.98% CAGR through 2030 as providers adopt sustainable transportation solutions. By application, dialysis transportation represented 29.42% of market size in 2024, while mental health visits record the highest forecast CAGR at 10.11% between 2025-2030.

Technology reshapes NEMT operations significantly. GPS tracking, fleet management systems, electronic health record integration, real-time route optimization, and predictive analytics help providers streamline operations and better anticipate patient demand. RouteGenie, a route-planning software, helps drivers maintain on-time performance by recording trip timestamps and tracking movements. NMotion equips drivers with company-issued tablets providing RouteGenie access, keeping staff informed about locations and maintaining scheduling precision.

In the United States, nearly 4 million people miss medical appointments annually due to transportation issues. Urban areas see highest demand for NEMT services, but rural regions offer untapped potential due to limited transportation options. That rural opportunity drives NMotion's expansion into northern Wisconsin, La Crosse, and Milwaukee—areas where transportation barriers significantly impact healthcare access.

The Regulatory Environment

NEMT providers navigate increasingly complex regulatory landscapes. Tighter Medicaid and CHIP (Children's Health Insurance Program) reporting requirements in 2025 create additional hurdles. Accurate coding and managing data across multiple systems have become more demanding, increasing administrative workloads and operational risks.

Many states now cut prior-authorization windows from 10 business days to 7 calendar days, forcing providers to invest in electronic verification systems that track pickup, drop-off, and mileage for up to six years. Oregon and New York adopted mandatory driver-certification schemes, increasing compliance overhead. Although these steps curb waste, they impose upfront costs that squeeze margins, particularly among small fleets lacking capital for audit-ready platforms.

Value metrics dovetail with CMS's 2026 rule that embeds transportation experience into Medicaid quality reporting, making technology prowess a prerequisite for long-term contracting. Performance-based reimbursement rules starting in 2026 raise the bar on on-time pickup, member satisfaction, and documentation quality across the NEMT market.

NMotion's credentialing process ensures compliance with federal and state requirements. The company's nearly decade-long run in the NEMT industry guarantees adequate resources and experience to navigate credentialing for fleet and drivers, including background checks, motor vehicle records, negative drug tests, medical checks, and training. The company can fulfill additional credentialing requests from different organizations and states.

The Expansion Opportunity

As NMotion expands statewide, it joins a consolidating industry landscape where technology-led brokers acquire regional operators to secure national reach and deeper data pools. However, opportunities remain for regional providers who understand local markets, build strong community relationships, and deliver consistent quality.

The company's expansion timing aligns with growing recognition that healthcare access requires reliable transportation infrastructure. Hospitals and healthcare systems increasingly view NEMT as essential to reducing missed appointments, improving health outcomes, and controlling costs associated with emergency department visits by patients who delay care due to transportation barriers.

Strategic partnerships with healthcare facilities, insurance companies, and managed care organizations provide NEMT providers with long-term contracts and stable revenue streams. NMotion's existing relationships with major Wisconsin healthcare systems position it well for expansion as these organizations extend services into underserved regions.

"Our work goes beyond driving. It's about connecting and supporting our community," Durruthy emphasized. As NMotion expands, it seeks individuals passionate about helping others while enjoying work-life balance. The combination of meaningful work, flexible scheduling, wellness focus, and growth opportunities creates an employment proposition designed to attract talent in a competitive labor market.

For Wisconsin residents seeking employment with purpose, opportunities to join the expanding NEMT sector offer pathways into healthcare support roles requiring no medical training. The work provides essential community service, stable income, and the satisfaction of directly improving patients' lives by ensuring they can access the medical care they need.
