Description
Fraser is hiring a full-time Program Coordinator for its Community Living team to oversee one Supportive Living apartment setting. The role provides leadership, supervision, coaching, and mentorship to staff; supports individualized treatment and service planning; coordinates intakes, discharges, and services with families and healthcare providers; develops staff training; maintains compliance with HIPAA, 245D, and Fraser policies; and participates in an emergency on-call rotation. The position is based in St. Paul or Minnetonka, Minnesota, with a Sunday–Thursday schedule and on-call rotation every six weeks, and pays $50,000–$53,000 annually plus a $1,000 hiring bonus for external new hires.
