Description
The Case Manager plans, coordinates, provides, and monitors therapeutic, rehabilitative, support, and community-integration services for adults with severe psychiatric disability. The role works independently, requires good clinical judgment, decision-making, time management, and appropriate treatment, and includes transporting clients as needed. Applicants need a bachelor’s degree in psychology, social work, psychosocial rehabilitation, or a related human-services field, or an associate’s degree in a related field plus two years of experience working with individuals with mental illness, along with an understanding of mental illness, a valid driver’s license, and the ability to transport clients.
