Description
The ACT Clinician delivers comprehensive and effective services to individuals aged 18 and above diagnosed with severe mental illness (SMI) and those with co-occurring substance use disorders. This role involves providing integrated treatment, rehabilitation, case management, and support services through a mobile, multidisciplinary mental health team. The clinician supports recipients' recovery by helping them achieve housing, employment, and stable relationships, using an individualized approach.
