The Activities Coordinator at AltaMed is a crucial member of the PACE Interdisciplinary Team (IDT), focusing on comprehensive care coordination. This role involves assessing and planning care, designing and implementing diverse activities to enhance the physical, cognitive, social, spiritual, and emotional well-being of PACE participants. The coordinator utilizes creative methods like arts, animals, sports, music, and community engagement, adapting programs using remote technology where appropriate. Adherence to PACE guidelines, regulations, and organizational policies is essential, operating under the supervision of the PACE Care Center Supervisor.
