Description
The Activities Coordinator at AltaMed is a member of the PACE Interdisciplinary Team (IDT) who participates in care coordination activities like assessments and care planning. They design, implement, and oversee engaging activities for PACE participants, focusing on physical, cognitive, social, spiritual, and emotional well-being. These activities include arts and crafts, animal interaction, sports, games, dance, music, and community outings. The coordinator ensures adherence to PACE program guidelines, regulations, and organizational policies, operating under the supervision of the PACE Care Center Supervisor.
