UW Health is hiring a full-time Advance Care Planning Program Coordinator to coordinate advance care planning activities and strategic initiatives across multiple healthcare stakeholders, develop and optimize workflows, monitor data, evaluate program outcomes, and support patient and provider communications. The role is based at the Administrative Office Building in Madison and may be performed remotely from home two to three days per week. A master's degree in a relevant field and five years of healthcare experience in advance care planning, care coordination, chronic disease management, bereavement, or palliative care are required; professional licensure is required, while RN, social work, chaplaincy, and advance care planning certifications are preferred.
