Description
The Advanced Certified Hospice and Palliative Social Worker (ACHP-SW) provides integrative psychotherapy, utilizing expressive arts therapy, to individuals, children, and families facing chronic or life-threatening illnesses. This role involves comprehensive assessments, counseling support for mental, emotional, social, and behavioral issues, and assisting patients in coping strategies and behavioral change. The ACHP-SW works collaboratively with healthcare professionals and administrative staff for care coordination and treatment planning, acting as the primary mental health resource. They also contribute to implementing best practices in integrative psychotherapy and guided imagery.
