Description

John Muir Health is seeking an Advanced Practice Pharmacist (APP) to manage patients with complex and chronic diseases in a Pharmacotherapy Clinic. This role involves prescribing medications, optimizing therapeutic outcomes, conducting clinical assessments, interpreting lab results, and collaborating with healthcare teams. The APP will also contribute to policy development, quality improvement, and staff education.

Additional Information

Minimum Education: Doctor of Pharmacy (PharmD) or Bachelor of Science (BSPharm) from a school accredited by ACPE
Required Previous Experiences: Board certified in a relevant clinical area.
PGY-1 or PGY-2 residency in Ambulatory Care, Pharmacotherapy, Cardiology, Internal Medicine, Oncology, Geriatrics, Specialty Pharmacy, or other applicable specialty.
1,500+ hours of direct patient care under CPA or similar protocols.
Preferred Qualifications: Five years of clinical experience in ambulatory care or chronic disease state management.
Ability to multitask and adapt to a high-acuity outpatient environment.
Demonstrated ability to perform in a complex and collaborative clinical setting.
Ability to educate, monitor, order, and follow labs for patients, related to metabolic disease states.
Experience in developing policies and engaging in continuous process improvement.
