Legacy Community Health is hiring a Nurse Practitioner - Transitional Care Management to provide advanced clinical care to patients transitioning from acute or post-acute settings to outpatient care. The role includes post-discharge assessments, medication reconciliation, diagnosis and management of acute and chronic conditions, care planning, patient and caregiver education, coordination with providers and community resources, follow-up tracking, and participation in quality and interdisciplinary activities. The position is scheduled Monday through Friday from 8:00 AM to 5:00 PM and requires a current Texas nursing license, completion of an accredited program, and two years of experience.
