Community Health Network is seeking a 0.6 FTE Advanced Practice Provider, either a Nurse Practitioner or Physician Assistant, to join its Palliative Care team in Indianapolis. The provider will work autonomously and collaboratively with multidisciplinary teams as a float provider across several network hospitals, delivering palliative and healthcare services, focusing on patient comfort, end-of-life care, symptom management, consults, and new admissions. The role requires 1–3 years of experience as a licensed Nurse Practitioner in palliative care, independent patient assessment, strong communication and critical-thinking skills, and the ability to educate patients and families.
