Description
High Point is hiring an Aftercare Coordinator for its High Point Inpatient Campus in Hyannis, Massachusetts. The role provides direct aftercare and discharge planning, maintains resident medical records, coordinates referrals and community services, facilitates groups and milieu activities, supports crisis de-escalation and safety monitoring, and completes required documentation. It is available full time, part time, or per diem across all shifts and pays $22–$27 per hour depending on education and experience. A high school diploma or GED is preferred, along with an associate's degree in human services and one year of experience in substance use treatment or mental health; CPR certification is also preferred.
