Description
The Dauphin County Area Agency on Aging is hiring a full-time, day-shift Aging Care Manager 1 at $20.00 per hour under a collective bargaining agreement. The role develops care plans for older adult consumers, evaluates their social supports, activities of daily living, cognitive and physical health, and other resources, and maintains relationships with social service agencies and community resources. The position is supervised by an Aging Care Management Supervisor 1 and requires a bachelor's degree or an alternative education and experience combination, along with a valid Pennsylvania driver's license, background checks, clearances, and favorable references.
