Pathways Vermont is hiring one Assertive Community Treatment (ACT) Team Services Coordinator for in-person work in Franklin County. The full-time role provides flexible, compassionate, community-based services to clients with mental health challenges, histories of homelessness, and substance use challenges, including service planning, housing navigation, referrals, benefits and entitlements support, life-skills counseling, recordkeeping, and coordination with community agencies. The position includes a rotating 24/7 on-call schedule and requires a valid driver’s license, reliable vehicle, automobile insurance, and the ability to lift and carry 50 pounds. Compensation is $22–$24 per hour with a $2,000 hiring bonus available, plus medical, dental, vision, retirement, paid time off, holidays, and other benefits.
