Breaking Ground is hiring an Assistant Director, Programs, Case Management to support case management and housing placement work for its Street to Home program in Queens. Reporting to the Director, Programs, the role supervises clinical, housing, reporting, and street medicine coordinators; provides structured supervision, crisis intervention, risk assessments, service planning, case conferences, and coordination with external agencies; and ensures Housing First, harm reduction, trauma-informed, and other evidence-based interventions are applied. The position works primarily during business hours but requires oversight of 24/7 operations and schedule flexibility, and requires an NYS Licensed Masters Social Worker, at least two years of supervisory experience, and relevant clinical and stakeholder-management skills.
