Assistant Social Services Director at Care One

CareOne is hiring a full-time Assistant Director of Social Services at its New Bedford facility. The role oversees the Social Service department, supervises staff, provides direct services, advocacy, counseling, service coordination, discharge planning, care planning, documentation, and community relations, and coordinates with residents, families, outside agencies, and interdisciplinary teams. Candidates need a master's degree in social work or human services, a CSW or LSW credential, and at least three years of social work experience; long-term care or skilled nursing experience is preferred.
