Description
Vynca is hiring a Behavioral Health Case Manager for its Enhanced Care Management team. The role serves as the client’s primary point of contact, coordinates care across medical, behavioral health, social services, housing, and other providers, develops care plans, advocates for members, connects clients to community resources, documents progress in EMR and CRM systems, and completes reporting. It is a hybrid position requiring travel throughout Humboldt County up to five days per week, with candidates required to reside within 20 miles of the assigned territory. The position requires an active, unrestricted California LCSW, LMFT, or LPCC license, at least one year of relevant care-management or community-health experience, and availability Monday through Friday from 8:30 a.m. to 5:00 p.m., with flexibility for evenings and weekends.
