Vynca is hiring a hybrid Behavioral Health Case Manager for its Enhanced Care Management team in Orange County, California. The role coordinates client care across physical, mental, substance-use, palliative, memory, and social-service needs; develops care plans; advocates with providers; connects clients to community resources; documents progress in EMR and CRM systems; and completes reporting. The position requires travel throughout the Orange County area up to five days per week, with candidates residing within 20 miles of the assigned territory. An active California LCSW, LMFT, or LPCC license and at least one year of relevant care-management experience are required, while bilingual English/Spanish ability is preferred.
