Vynca is seeking a remote Behavioral Health Case Manager, also known as a Clinical Lead Care Manager (CLCM), to manage client cases and coordinate healthcare benefits. The role involves assessing member needs across various aspects of well-being, developing care plans, connecting clients to necessary services, advocating for patients, utilizing evidence-based practices like Motivational Interviewing, conducting outreach, evaluating progress, providing mental health promotion, arranging transportation, completing documentation, maintaining EMR records, and performing monthly reporting.
