Description
Vynca is hiring a hybrid Behavioral Health Case Manager for its Enhanced Care Management team in Los Angeles County, California. The role coordinates client care across physical, mental, substance-use, palliative, memory, and social-service needs; develops care plans; connects clients to providers and community resources; advocates for members; conducts outreach; documents progress in EMR and CRM systems; and completes reporting. The position requires travel throughout the Los Angeles County area up to five days per week, with candidates residing within 20 miles of the assigned territory. An active California license in ACSW, LCSW, LMFT, or LPCC is required, along with at least one year of relevant care-management or community-health experience; bilingual English/Spanish ability is preferred. The role offers a $2,500 sign-on bonus paid in installments if the offer is signed by September 30, 2026.
