Description
Heartland Family Service is hiring an Assertive Community Treatment Case Manager to serve individuals with serious mental illness in rural Southwest Iowa counties. The role provides comprehensive assessments, client-centered treatment planning, skills development, crisis intervention, coordination with community agencies, documentation, and support to clients, families, and team members. It requires a bachelor’s degree in a human service field, three years of case management, vocational, or skill-development experience, a valid driver’s license, and an acceptable driving record; experience with serious and persistent mental illness is preferred. The position is scheduled 40 hours per week, Monday through Friday, 8:30 a.m. to 5:00 p.m., with compensation of $40,800 to $51,606 per year.
