Bereavement Coordinator at Compassus

Description

The Bereavement Coordinator at Compassus is a hospice-trained interdisciplinary team member responsible for developing and delivering a comprehensive bereavement support and counseling program. This includes providing grief counseling for patients during hospice care and for bereaved family members for at least one year post-death. The coordinator works collaboratively with the hospice Social Worker, identifies bereavement risk factors, develops care plans, educates the IDT on bereavement services, prepares documentation, and engages in community outreach and memorial services.

Additional Information

Required Previous Experiences: Two (2) years of experience in the provision of grief, loss and/or bereavement support for individuals and families.
Preferred Qualifications: Experience and/or education in dealing with issues of grief and loss.
Experience or education in counseling individuals dealing with issues of grief and loss.
Licensed professional counselor (LPC); or Licensed social worker (LMSW, LCSW); or education/experience specific to the counseling of individuals dealing with grief and loss.
Completed 8 additional hours in death, dying, grief, or bereavement.
