SnappyCX is hiring an experienced Care Coordinator for a fully remote, full-time role supporting a high-volume home care environment. The position serves as a central contact for patients, families, caregivers, referral sources, and healthcare professionals; coordinates referrals, appointments, care plans, provider follow-ups, escalations, and staff assignments; and maintains accurate client records. Candidates should have prior home care coordination experience, familiarity with home care scheduling software and EHR systems, strong communication and organizational skills, knowledge of HIPAA and state home care regulations, and proficiency with Microsoft Office.
