Pair Team is hiring a Field Care Coordinator to provide high-volume, in-person care management and care coordination for complex Medicaid and Medicare patients across Alameda County. The role involves home and community visits, new patient onboarding, care plan follow-up, health assessments, referrals, resource connection, documentation, and collaboration with nurses and behavioral health care managers. It requires bilingual English/Spanish communication, at least one year of field-based case management or care coordination experience, reliable transportation, a valid driver’s license, and strong organizational and technical skills. The schedule is Monday through Friday, 9:00 AM to 5:30 PM, with approximately 30 visits per week and 30–40 miles of travel daily.
