Bilingual Field Care Coordinator / Field Case Management at Pair Team

Pair Team is hiring a full-time Field Care Coordinator for a field-based care management pilot in Orange County. The role provides in-person support to members with complex medical, behavioral, and social needs, including home and community visits, care plan follow-up, care coordination, referrals, documentation, and advocacy. The position requires English/Spanish fluency, at least one year of field case management or care coordination experience, reliable transportation, a valid driver’s license, and HIPAA-compliant documentation capabilities.
