Description
Pair Team is hiring a full-time Field Care Coordinator for a field-based care management pilot in Orange County. The role provides in-person, relationship-based support to members with complex medical, behavioral, and social needs, including home and community visits, care plan follow-up, care coordination, referrals, documentation, and advocacy. The position requires English/Spanish fluency, at least one year of field case management or care coordination experience, reliable transportation, a valid driver’s license, and HIPAA-compliant documentation capabilities. The schedule is Monday through Friday, 9:00 a.m. to 5:30 p.m., with four days in the field and one work-from-home day, and pays $30 per hour with health, vision, dental, retirement, stipend, and reimbursement benefits.
