SnappyCX is hiring experienced Patient Coordinators for a fully remote, full-time position supporting home-care clients. The role coordinates communication among contracts, patients, families, home-health aides, contractors, clinicians, and other care-team members; schedules work assignments; documents communications in HHA Exchange; manages cases, reports, overtime, travel, and incident reporting; and responds to emails, voicemails, and faxes. Bilingual fluency in English and Spanish is required, along with strong customer service, communication, multitasking, time-management, and adaptability skills.
