Description
East House is hiring a Health Home Care Manager to provide comprehensive care management, care coordination, client advocacy, crisis intervention, safety planning, documentation, and community outreach for individuals recovering from mental illness, chemical dependency, and physical illness. The role manages a short-term caseload of 25–30 clients, coordinates services across providers, supports transitions, and collaborates with multidisciplinary teams and community partners. A high school diploma or GED and 1–3 years of case management experience are required; an associate’s degree in a related field and Narcan training are preferred. The position is based in a traditional office environment, may require travel to program locations, and pays $18.00–$22.85.
