Sun River Health is hiring a full-time Care Coordinator for its administration sites in Peekskill and Tarrytown, New York. The role supports patients with preventive and guideline-concordant care by coordinating follow-up services, helping overcome appointment and insurance barriers, collecting and entering cancer screening outcome data in the Catalyst system, communicating with healthcare providers and community organizations, and coaching patients to advocate for their care. A high school diploma or GED is required, a bachelor's degree is preferred, and a driver's license is required. The hourly pay range is $24.00 to $27.35.
