Bluestone is hiring a full-time Care Coordinator to manage DHS and CMS-required activities for Minnesota Senior Health Options members living in the community and assisted living settings. The role is primarily field-based with substantial local travel throughout Ramsey County/St. Paul and partial remote work, and involves face-to-face visits, assessments, care planning, resource coordination, documentation, and collaboration with medical care teams. The posting requires either a current Minnesota Licensed Social Worker license or a Minnesota Registered Nurse license, a valid driver’s license, and preferred experience in geriatric or disabled-population case management/care coordination.
