I Am Boundless is hiring a Care Coordinator to assess needs, link services and resources, and coordinate care for youth and families or caregivers in the OhioRISE plan. The role is primarily community-based but may include some home-based work, and involves coordinating with individuals, parents or guardians, Boundless staff, community members, and service providers through face-to-face, telephone, video, and electronic communication. The position requires a bachelor’s degree in psychology, social work, or a related field, or at least three years of relevant experience, along with specified driver, insurance, training, and first-aid/CPR qualifications.
