Intermountain Centers is recruiting a Care Coordinator to manage members’ whole health through an integrated case-management model. The role assesses, plans, implements, monitors, and evaluates services; documents observations and clinical records; coordinates with members, families, providers, and interdisciplinary teams; develops individualized service plans; and advocates for access to resources. The position requires a behavioral-health-related bachelor’s degree or an H.S. diploma plus two years of behavioral-health experience, along with age, fingerprint, CPR/First Aid/AED, driver’s-license, and other regulatory requirements.
