Description
The Care Coordinator provides person-centered care management for adults and children with chronic behavioral health and medical conditions. The role assesses needs, develops care plans, coordinates medical, behavioral health, social, and community services, tracks referrals in an electronic health record, responds to member crises, and provides outreach. It is primarily field-based in New York City, with office-based work and travel to appointments, and requires a relevant bachelor’s degree or equivalent education and experience, interdisciplinary care-management experience, and proficiency with EHR and Microsoft Office.
