Lifekind Health is hiring a full-time Care Coordinator in Victorville to assess patient needs, develop individualized care plans, coordinate appointments and follow-up, serve as a patient and family liaison, facilitate communication among healthcare providers and community resources, monitor progress, and address barriers to care. The role requires at least two years of experience in care coordination, case management, or a related healthcare role, a high school diploma or equivalent, and proficiency with electronic health records; Saturday and/or Sunday availability is required.
