Segue Health is hiring a full-time Care Coordinator to support post-discharge transitional care management for patients in DeSoto County and surrounding areas. The role coordinates referrals, documents discharges, creates EMR charts, makes post-discharge calls, schedules in-home visits, and communicates with referring facilities. Candidates need medical-field experience, Microsoft Excel knowledge, EMR/EHR learning ability, transportation to local hospitals, and strong organizational and communication skills; social work, nursing, or home health experience is preferred. The position offers an 8-hour Monday-to-Friday schedule and a salary of $45,000 to $55,000 per year.
