Segue Health is hiring a full-time Care Coordinator to support transitional care management for patients in the Texas Health Presbyterian Dallas Hospital area. The role coordinates referrals, documents discharges, creates EMR charts, makes post-discharge calls, schedules home visits, and works with referring facilities to identify eligible patients. Candidates need medical-field experience, Microsoft Excel knowledge, EMR/EHR learning ability, transportation, and strong organizational and communication skills; social work, nursing, or home health experience is preferred. The position offers an 8-hour Monday-to-Friday schedule and a salary of $50,000-$60,000 per year.
