Segue Health is hiring a full-time Care Coordinator to support post-discharge transitional care management in the Knoxville, Tennessee area. The role coordinates referrals, documents discharges, creates EMR charts, makes post-discharge calls, schedules home visits, and visits referring facilities. Candidates need medical-field experience, Microsoft Excel knowledge, EMR/EHR learning ability, transportation, and strong organizational and communication skills; social work, nursing, or home health experience is preferred. The position offers an 8-hour Monday-to-Friday schedule and pays $45,000-$55,000 per year.
