The role involves identifying eligible patients, visiting referring facilities, logging referrals and discharges, creating EMR charts, making post-discharge calls, and scheduling in-home visits. Candidates need medical-field experience, Microsoft Excel knowledge, EMR/EHR learning ability, transportation, and strong organizational and communication skills; social work, nursing, or home health experience is preferred.
