SnappyCX is looking for experienced Care Coordinators to join their client's care team. This is a fully-remote, full-time position where you'll act as the main point of contact for patients, families, clients, and care staff in a fast-paced, high-volume home care setting. The role involves handling numerous incoming calls, coordinating care, scheduling appointments, reviewing doctor's notes, confirming care plans, monitoring assignments, logging updates, tracking staff, ensuring patient attendance, following up with providers, maintaining client records, conducting provider follow-ups, and communicating with various parties.
