The Care Coordinator supports women in a residential co-occurring substance use and mental health treatment program by coordinating medical and behavioral health services, referrals, medication-assisted treatment, psychiatric care, and aftercare. The role also provides milieu coverage, maintains client records, collaborates with staff, and attends weekly supervision. It is a full-time, on-site position in Jamaica Plain, Massachusetts, with a stated rate of $26.00 per hour and benefits including employer contributions toward health and dental insurance.
