NYPCC is hiring a full-time Care Coordinator for its Child and Family Health Center in Jackson Heights, New York. The role manages an 85+ caseload of Health Home Care clients, develops comprehensive care plans, coordinates housing, benefits, medical, transportation, education, employment, crisis intervention, and other supportive services, and maintains advocacy, referrals, documentation, and electronic health records. The position requires a bachelor's degree, RMA experience, and English-Spanish bilingual ability; HARP-client experience is preferred. The hybrid schedule is Monday through Friday, with onsite work on Monday, Tuesday, and Thursday. The salary is $47,000 to $60,000 per year, with medical, dental, vision, paid time off, retirement, and other benefits.
