Description
The Care Coordinator provides case management, participant outreach, intake, enrollment, assessment, service planning, referrals, and follow-up for families and service participants. The role requires a high school diploma or equivalent, at least three years of related professional experience, First Aid and CPR certification, a valid Texas driver’s license, reliable transportation, and work authorization in the United States. It is a full-time position requiring travel within the service area and participation in Salesforce and PEIRS documentation.
