Description

Harbor is hiring a full-time Care Coordinator to provide comprehensive, multi-system care coordination for youth with complex behavioral health needs and their families. The role is remote and in the community, requires at least three years of relevant experience or equivalent education-based experience, and includes caseload management, resource linkage, CANS assessments, care planning, family-team meetings, crisis planning, clinical consultation, documentation, and home-based services. Candidates must have a valid driver’s license, acceptable driving record, and obtain CPR/First Aid and NCI/CPI certifications within 90 days of employment.

Additional Information

Required Previous Experiences: At least 3 years of experience providing community-based services and supports to children, youth, and families in behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or related public sector human services or behavioral health care.
Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
Training and experience managing complex cases in a culturally competent manner.
Experience working with children and families experiencing substance use disorders, trauma, co-occurring behavioral health disorders, and engagement with child-service systems.
Completion of the high-fidelity wraparound training program provided by an independent validation entity recognized by the ODM (the COE).
