Description
The Care Coordinator serves as a central communication and care-management hub for patients, families, physicians, specialists, and administrative staff. The role manages care plans, electronic health records, referrals, insurance eligibility, patient inquiries, community resources, treatment adherence, and care gaps while maintaining HIPAA compliance and escalating operational issues. It is available full-time or part-time in a remote setting and requires at least three years of experience in US healthcare coordination, medical administrative support, patient navigation, or case management.
