Harbor is hiring a full-time Care Coordinator to provide comprehensive, multi-system care coordination for youth with complex behavioral health needs and their families. The role is remote and in the community, with candidates preferably residing in Ottawa or Sandusky. Responsibilities include engaging youth and families, building community-resource knowledge, conducting CANS assessments, developing and monitoring individualized care plans, facilitating Child and Family Team meetings, coordinating clinical and psychiatric consultation, developing safety-crisis plans, and completing required documentation. The position requires at least three years of relevant experience or equivalent education, wraparound training, strong communication and computer skills, a valid driver’s license, and CPR/First Aid and NCI/CPI certifications within 90 days.
