Description
The Greater Lawrence Family Health Center is hiring a bilingual Care Coordinator for its Recovery Based Re-Entry Services for Black and Latino Men Program. The role coordinates culturally responsive care and community resources for individuals transitioning from House of Corrections, develops individual service plans, supports reintegration activities, assists with eligibility and agency requirements, provides low-threshold substance-use interventions, and links clients to appropriate services. The position requires Spanish/English bilingualism, a valid Massachusetts driver’s license and reliable transportation, and experience with priority populations; community health worker and recovery coach certifications are required or must be completed within 12–18 months of hire.
