Description
The Care Coordinator, RN provides care coordination services in a primary care practice for high-risk members and people facing healthcare barriers. Duties include comprehensive assessments, member-centered episodic and longitudinal care planning, monitoring facility stays and discharges, disease education, crisis intervention, motivational interviewing, self-management support, referrals to community resources, documentation in care management systems, collaboration with providers and care teams, and support for population management and quality improvement. The role requires a valid RN license, at least one year of case management or relevant healthcare/behavioral health experience, and may involve remote work with Colorado licensure, a compliant home office, and insured transportation if applicable.
